Transitioning care from the hospital to the home is important to relieve the capacity in healthcare. More importantly, patients experience this as more pleasant. For over 20 years, our AmgenCare care programs have helped support many patients in their homes by self-administering or receiving medication. Additionally, we offer support to hospitals in transitioning care to patients' homes.
Helping healthcare providers deliver the best patient care and sharing knowledge and experience are of great importance to us. That's why we're involved in the CareCoalition*. The CareCoalition is a platform for initiatives and knowledge sharing in the field of transitioning hospital-based care to the home. Here, we join forces to further relieve the pressure on healthcare, with specific attention to hospital-relocated oncological care. We prioritize and collaborate in the implementation of the Integral Care Agreement (IZA) and help hospitals realize suitable care closer to home or at home, without having to reinvent the wheel every time.
